Lung Regeneration and Repair

Objectives

Diseases of the respiratory system are one of the major causes of deaths worldwide. For the most common terminal respiratory diseases, including chronic obstructive pulmonary disease, pulmonary fibrosis, arterial pulmonary hypertension, and cystic fibrosis, lung transplantation (DLTx) remains the only curative therapy option. Within this collaborative project we aim at the development of alternative therapy strategies to the DLTx, including search for regenerative drugs, cellular therapies and ex vivo treatment.

Efficient protocols for specific and scalable differentiation of human induced pluripotent stem cells (iPSCs) into functional endothelial, alveolar, and bronchial epithelial cell lineages are developed and the respective cell lineages are utilized for the treatment of various respiratory diseases. Human reporter iPSC lines are available or are under construction that allow monitoring of differentiation processes and will facilitate disease modelling, screening for key differentiation factors and regenerative drugs, the development of advanced scalable differentiation protocols and enrichment of the desired pure endothelial and respiratory/airway stem/epithelial cell population by FACS/antibiotic resistance. iPSC-derived respiratory progenitors and endothelial cells will be used for cellular therapies aiming towards lung regeneration and repair. This also includes ex vivo therapies in the the Organ Care System (OCS), enabling explanted lungs to be perfused and ventilated in a nearly physiological setting, and to be treated outside the body. iPSC-derived endothelial cells produced in defined scalable processes are also applied for seeding of a biohybrid lung.

Long term goals

  • Development of novel small molecule drugs for treatment respiratory disease including cystic fibrosis (CF) and pulmonary arterial hypertension (PAH)
  • Development of new therapeutic strategies targeting chronic obstructive pulmonary disease (COPD)
  • Functional repair of diseased lung tissue by cellular therapies (e. g. pulmonary arterial hypertension)
  • Engineering bioartificial lung tissue substitutes and lung-on-the-chip systems
  • Ex vivo gene therapy, lung regeneration and repair using the OCS

Research Focus

As aforementioned, for many patients, lung transplantation (DLTx) remains the only effective therapeutic option. However, DLTx carries several inherent drawbacks: (1) Only few patients are eligible for transplantation because of factors such as age and co-morbidities, (2) there is still a substantial shortage of donor organs, (3) patients with end-stage lung disease often become critically ill and may not survive until a donor organ is available, and (4) organ transplantation generates its own array of new medical problems and as a result, life-expectancy after this operation remains limited.

Pluripotent stem cells have hardly been applied for modelling respiratory diseases, to screen for novel regenerative drugs or for development of cellular therapies targeting lung diseases. Notably, these cells allow for ex vivo correction of genetic defects, followed by their targeted differentiation into the desired cell type such as endothelial cells and respiratory epithelial cells, which may finally allow reconstitution of disease-free respiratory tissue.

Novel technologies for engineering of (bio)artificial respiratory tissue substitutes and lung-on-the-chip assays represent innovative systems for the development of new personalized drugs. Moreover, the development of an implantable biohybrid lung and concepts for ex vivo treatment of isolated lungs may represent promising ways to reduce the number of necessary DLTx.

Within REBIRTH, Unit 4.2, we aim to develop new regenerative drugs and iPSC-based therapies for the treatment of diseases of the respiratory system. This includes the correction of genetic defects in patient-specific iPSCs, the development of improved protocols for targeted differentiation of iPSCs into endothelial cells and respiratory cell lineages, and large scale production of sufficient amounts of cells. Finally, such cells could be utilized for modelling lung diseases in a dish, for high throughput screens, biofunctionalisation of biohybrid lungs, ex vivo cell therapy using the OCS, and application in preclinical animal models.
